The blade stands on four legs, which have the shape of a curled blade / C volute. The edge is decorated with engraved top / curl ornament. Two arms are engraved on the center of the bottom: on the left the city coat of arms of Leiden [shield with crossed keys] and on the right the coat of arms of the St. Catharinagasthuis [stylized floral pattern in circles with crown, on the left and right the letters KG, the outer edge is decorated with leaves]. Under the arms are two crossed branches with leaves. The inscription is engraved on the bottom: The City Council of Leiden To the Lord W.J. Hoefman City Pharmacist: For extraordinary service During the prevailing diseases in 1859 LEAD May 24, 1860 .. Marked on the bottom, 1915.
